First, when you arrive to our chiropractic office in Ozark, we’ll ask you to fill out our new patient intake forms. This paperwork provides us basic information about you, as well as your medical history and a description of your condition. You can fill these forms out ahead of time by going here.
Next, you’ll have consult with Dr. Zach Fish to chat about your health-related problems and concerns, as well as potential treatment options. This initial visit is designed for Dr. Zach Fish to understand exactly where you are at with your health and provide a roadmap to get you feeling your absolute best.
After your initial discussion, Dr. Zach Fish will perform a complete chiropractic exam. This is to get hands-on and see exactly what is going on with your body currently. Nothing will be done without full explanation and your consent.
4. Report of Findings
After the exam is completed and analyzed, Dr. Zach Fish will give you a “report of all findings” that details everything discovered during the exam. After reviewing your health history, goals, and examining your spine and X-rays, Dr. Zach Fish will discuss recommendations for your treatment in order to help get you to optimal health.
At the end of your first appointment, you can choose to experience your first chiropractic adjustment. This may include spinal adjustments, intersegmental traction, and/or spinal decompression. Treatment is interactive, so you can express concerns about the different styles of treatment anytime.
6. Custom Wellness Plan
After your first treatment, our team will determine if ongoing wellness and care will make sense for your health. These plans are designed to help maintain optimal health for your spine and your body.
Keeping on top of your spine’s misalignments helps prevent future issues from arising and keeps you feeling great.

For me, we added Soft Wave treatments for my knee recovery a couple years after a major accident. I no longer have any knee pain associated with the accident, and I have beyond full range of motion. My son is currently receiving Soft Wave on his shoulder and is majorly improved, and both of our daughters are now free of neck and back pain. Dr. Fish stays relevant and is constantly continuing his education in chiropractic care and functional living, and teaching that to his clients and community. I started seeing Dr Fish with terrible back pain, neck stiffness. It was horrible walking and standing.
I would wash dishes after dinner and have to take a break after just a few minutes. That has been fixed (much to the pleasure of my wife).
The appointments are fast and effective.
First you go to a roller bed that heats and rolls up and down your back, it great!
Then you go in another room and Dr Fish comes in and adjust your back and neck.
Whole visit is fast, no taking up your time just sitting around waiting.
Jaden and Bailey who work the front desk are always friendly and happy to see you.
All in all I couldn’t ask for a better chiropractor.

I had been experiencing arthritis in my right knee for about a year. Ortho immediately wanted to do a knee replacement. I opted to try Softwave first. I have gone from almost zero mobility to 2 mile walks. I am not 100% pain free but I’m at the end of my treatment and staff tells me the healing continues even after treatments end. Staff at restore is excellent. Very friendly and helpful. Good communications with appointments.
Im about half way through my treatments, and can already feel a big difference! The staff is great, very personable. I really appreciated Dr. Zach listening and coming up with a treatment plan that targeted all my concerns, and then checking in each visit to make sure we are moving in the right direction.So happy with my progress! Restore Family Chiropractic has been helpful from my first appointment to create a plan to help me find pain relief for my chronic back pain. Within the first couple of weeks I’ve felt a difference and my pain has decreased significantly. I’d recommend Restore to anyone who is seeking pain relief!

Honestly, the care I have received here is top notch from the very beginning assessment to day to day staff interactions, there is no place like Restore in the Ozark/Nixa area. Highly recommend going to see Zach F. The team is helpful. I didn’t have any trouble getting appointments booked twice weekly for the first month. I do wish they would do itemized billing to turn into insurance. Restore does not bill insurance but, I could turn in my itemized bills if they were an option. I would have given them five stars if they would bill insurance or do an itemized bill. I know the team does care about our community and their clients by the back to school supply drive and the family day offered to us. I love going to a local business that gives back!
